我院门诊中药饮片处方点评及分析(1)

     【摘要】 目的 分析本院門诊中药饮片处方中存在的问题, 并提出规范处方建议或措施, 以期为指导中药房调剂工作和促进临床合理用药提供科学依据。方法 采用随机抽样法从本院2016年1~12月门诊中药饮片处方中抽取780张, 根据相关标准对处方进行点评分析。结果 抽查的780张中药饮片处方中, 涵盖了本院19个科室, 其中中医内科253张(32.44%), 妇科106张(13.59%), 泌尿科74张(9.49%);平均每张处方用药味数(16.1±5.4)味。780张中药饮片处方中发现不合理处方247张(31.67%), 主要类型包括不规范处方(中医诊断不规范, 未使用药品规范名称, 未按要求标注药物调剂、煎煮等特殊要求, 门诊处方超过7 d用量未标明原因)197张(79.76%)和不适宜处方(适应证不适宜, 用法不适宜, 药物剂量不适宜, 存在配伍禁忌或不良相互作用, 重复用药, 超常处方)50张(20.24%)。结论 本院门诊中药饮片处方仍存在不合理用药情况, 应建立健全处方点评制度, 完善电子处方系统, 加强中医师和药师专业培训, 以保证临床用药的有效性和安全性。

    Comments and analysis of prescriptions for Chinese herbal decoction pieces in our hospital HUANG Xuan. Department of Pharmacy, Zhaoqing Second People’s Hospital, Zhaoqing 526040, China

    【Abstract】 Objective To analyze the problems existing in the prescription of Chinese herbal decoction pieces in outpatient department of our hospital, and put forward suggestions or measures to standardize the prescription, in order to provide scientific basis for guiding the dispensing work of Chinese herbal pharmacy and promoting the rational use of drugs in clinic. Methods A random sampling method was used to extract 780 samples from the prescriptions of Chinese herbal decoction pieces in our hospital from January to December 2016, and the prescriptions were commented and analyzed according to relevant standards. Results The 780 prescriptions of Chinese herbal decoction pieces were sampled, covering 19 departments of the hospital, including 253 pieces (32.44%) in internal medicine, 106 pieces (13.59%) in gynecology and 74 pieces (9.49%) in urology, with average number of drugs per prescription as (16.1±5.4) kinds. 247 pieces (31.67%) of unreasonable prescriptions were found in 780 pieces of Chinese herbal decoction pieces, including 197 pieces (79.76%) of non-standard prescriptions (diagnosis of traditional Chinese medicine was not standardized, the standard name of drug was not used, the special requirements of drug dispensing and decoction were not marked as required, and the reasons for the dosage of outpatient prescriptions exceeding 7 d without indication) and 50 pieces (20.24%) of inappropriate prescriptions (inappropriate indications, inappropriate usage, inappropriate dosage, incompatibility taboos or adverse interactions, repeated use of drugs, extraordinary prescriptions). Conclusion There are still unreasonable drug use in outpatient prescriptions of Chinese herbal decoction pieces in our hospital. In order to ensure the effectiveness and safety of clinical medication, the prescription evaluation system should be established and improved, the electronic prescription system should be improved, and the professional training of traditional Chinese medicine practitioners and pharmacists should be improved and strengthened., 百拇医药(黄轩)
